• Upon appointment, please inform us of insurance company, patient's name, date of birth, date and time, and reanson for visit.

      예약하실 때에는 건강보험회사, 환아의 이름, 생년월일, 원하시는 시간과 날짜, 진료 내용

      등을 말씀해주세요.

 

• For new patient, we need your insurance card and kid's immunization record. Of note, please bring the discharge summary for newborn infant or hospitalized patients.  

​      처음 방문하시는 경우에는 보험카드, 예방접종카드를 꼭 지참하시기 바랍니다. 특히 병원

      에서 퇴원하는 환아나 새로 태어난 신생아의 경우에는 병원에서 발급하는 퇴원요약지도

      함께 가져오시기 바랍니다.
